It is necessary to complete the online registration before making your payment for the conference.

To pay by credit card:

• Go to the following website: https://www.thepayplace.com/mi/dch/conference
• Select this conference code: "Imms08"
• Enter your name.
• Choose Next and complete your payment information.

To pay by check made payable to the State of Michigan:

The check should only include payment for the conference fees (e.g., payments for vaccine should not be included in your check). A copy of your registration confirmation must accompany your payment.

Payment Amount:

• The registration fee is $50.00
• The early registration fee ($50) will be available until 9/26/08.*
• Payment must be sent as soon as possible.

*After 9/26/08, the registration fee will increase to $75
